New Order Printer App Code Help

Hi, like a lot of people I had a great custom order template that now is missing two small pieces that I would love some help getting back. Is anyone that is familiar with the Order Printer Legacy/New App willing to help an old guy out with two simple changes?

The order template used to show the Order date and the Order number, now those dates and numbers are missing, and it used to show the Ship to address but that is all missing.

Any help is greatly appreciated.
