Shopify Forms APP - Add Phone number input

Hi! I am keen to use the new shopify forms app in my dawn theme.
However I require a phone number input field to be added. Is there any custom code I can use to include this? Thank you.

Hi thanks so much! However, I am looking to use the new Shopify Form App specifically as it pops up when somebody leaves the website. I don’t need an embedded contact form
