Manage forms

After you create a form, you can take actions to manage your forms from the Forms overview page, such as changing the status from active to inactive, renaming, or deleting a form. You can also view analytics about your forms and tag customers to create customer segments.

For an active form to display on your store, you need to have the Forms theme app embed installed on your store.

Activate a form

After you create a form, its status is inactive, and it doesn't display on your online store until you activate it. You can also activate a form that you've previously deactivated.

Steps:

  1. From your Shopify admin, go to Apps > Forms.

  2. Click … beside the form that you want to activate.

  3. Click Turn on.

Forms can also be activated from a specific form's page. Click the form that you want to activate, and then click Turn form on.

Deactivate a form

  1. From your Shopify admin, go to Apps > Forms.

  2. Click … beside the form that you want to deactivate.

  3. Click Turn off.

Forms also can be deactivated from a specific form's page. Click the form that you want to deactivate, and then click Turn form off. Forms don't display on your store when they're deactivated.

Rename a form

  1. From your Shopify admin, go to Apps > Forms.

  2. Click … beside the form that you want to rename.

  3. Click Rename form.

  4. Enter the new name for the form.

  5. Click Save.

Forms can also be renamed from a specific form's page. Click the form that you want to rename, click More actions, and then click Rename.

Delete a form

  1. From your Shopify admin, go to Apps > Forms.

  2. Click … beside the form that you want to delete.

  3. Click Delete form, and then click Delete form.

Forms can also be deleted from a specific form's page. Click the form that you want to delete, click More actions, click Delete, and then click Delete form.

Turn on email notifications for a form

If you want to receive an email notification each time a customer submits a form, then you can turn on notifications in your Forms settings.

If you want to send an email notification to multiple email addresses, then you can create an automation using Shopify Flow that sends an internal email to the recipient of your choice after a form submission.

When you receive an email notification about a form submission, the reply-to address is the customer's email address. If the customer signs up using only a phone number, then the reply-to address is no-reply@shopify.com.

Steps:

  1. From your Shopify admin, go to Apps > Forms.

  2. Click the form that you want to manage notifications for.

  3. In the Email notifications section, check Send an email for each submission to this form.

  4. Click Save.

Turn off Sign in with Shop on a form

When your store has Sign in with Shop activated, eligible forms display a Claim with Shop button by default. If you don't want the button to display on a specific form, then you can turn off Sign in with Shop for that form. This doesn't impact Sign in with Shop features on the rest of your store.

Steps:

  1. From your Shopify admin, go to Apps > Forms.

  2. Click the form that you want to edit.

  3. Turn off the Sign in with Shop option.

Forms analytics

You can view analytics about your forms from the Shopify Forms app Overview page in your Shopify admin. This page displays a metrics bar at the top of the page with aggregate data for all your forms, and an index table of metrics for each of your forms.

Aggregate forms data

  • Total views: The total number of views across all forms.
  • Total submissions: The total submissions from all forms.
  • Average completion rate: The total number of forms submissions divided by the total number of views, displayed as a percentage.

Individual form metrics

  • Views: How many times the form has been viewed.
  • Submissions: How many times a customer has successfully filled out and submitted the form. Learn more about viewing form submissions.
  • Completion rate: The number of form submissions divided by the total number of views.

By default, the overview displays all of your forms, but you can filter the view to display only the Active or Inactive forms. You can take actions to manage your forms from the main app page, such as activating, renaming, or deleting a form.

View form submissions

After a customer submits a form, the form submission data is stored in your Shopify admin. A customer profile is created if they're a new customer. Existing customers have their information matched to their profile with no modification to their email or phone number.

You can view the submissions for a specific form in the Forms app.

Steps:

  1. From your Shopify admin, go to Apps > Forms.

  2. On the Overview page, click the title of the form that you want to view.

  3. Click View submissions.

You can also view form submission data in the following locations:

  • In the Metafields section of a customer's profile
  • On the Metaobjects page of your Shopify admin, by clicking More actions > View read only definitions

Create a customer segment

You can create a customer segment for each of your forms. After you create a customer segment from a form, you can use the segment to send an email to those customers, or give those customers a discount.

Steps:

  1. From your Shopify admin, go to Apps > Forms.

  2. Click the form that you want to create a customer segment for.

  3. Click View segment.

  4. Optional: In the New segment field, apply any additional filters that you want to add to the customer segment.

  5. Click Apply, and then click Save segment.

  6. On the Save as new segment dialog, in the Segment name field, enter a name for your segment, and then click Save.

  7. Optional: Click Use segment, and then follow the prompts to send an email or discount to the customers in your segment.
